Ostrava appearing twice bug

I left him unrescued in 1-1. I killed Phalanx, then went to the Nexus. Reentered 1-1, killed Ostrava and went to the Nexus again, and Ostrava was sitting there (I had not gone into 1-2 yet, he would have reappeared there if I had). I didn't even have to talk to the Maiden in Black or the Monumental. He appeared in 1-2 as well when I got there. After this he should pop up in his usual places throughout world 1, and he'll drop yet another Mausoleum Key in 1-4, or when killed.

So you have to go by the Nexus after killing Phalanx, then by the Nexus again after killing Ostrava, before he reappears/revives. It also looks like he must be rescued and give the Brass Telescope gift before he dies, for the bug to occur.

• I have also killed him in 1-1 after defeating Tower Knight, and once after beating Penetratror, and he was in 1-2 both times.
• I have also tried before killing Phalanx, but couldn't get him to reappear in 1-2.
• I killed Phalanx, then went to the Nexus. Reentered, killed Ostrava and went straight to where he appears in 1-2, but he wasn't there.
• I killed Phalanx, then Ostrava, and went to the Nexus. Reentered in 1-2, but he wasn't there.
• Also, all of Patches' unlockable items should be available when Ostrava dies the second time.

Ostarva stuck in the pit

Had a new experience today that may explain why some folk have reported that Ostrava disappears and therefore they lose the Mausoleum Key.

1-3 NG

1. Saved Ostrava in 1-1 and 1-2.
2. Opened the portcullis and waited for Ostrava and foe's to drop while chucking Soul Remains.
3. He dropped into the recessed pit on the farside, from where I eventually heard his death rattle.
4. Two Red Eye Knights remained in the pit and could be heard clanking about.
5. Made my way back up on top of the portcullis. I could see from where the three Crossbow Soldiers stand, that Ostravas drop was in the pit.
6. Hopped over the wall, dropped onto and across the low wall directly above the portcullis to get to the pit.
7. Killed the two Red Eye Knights in the pit and picked up the Mausoleum key.
8. Used a Splinter of Archstone to return to the Nexus.

Twin Fangs inconsistency?

Ostrava says that Vallarfax was lost (killed by Miralda most likely) and that Biorr slipped through the fissure never to be seen again ( He's behind the fog/fissure that only goes away after killing an archdemon locked in jail). He never says that Vallarfax goes back after warning the world so where exactly is the inconsistency?
